Your Living Name
On this form we ask for your living name, not your legal name as it appears on government documents. Your living name includes your given names and your house name. Your house name incorporates both your mother's original house and your father's house. Fill in each part separately.
All names you were given at birth before your family name. This includes what is commonly called a middle name. In your living name all names before your family name are simply your given name.
Your mother's original house name. The house she was born into, before any later married or legal surname.
Your father's house name. The house passed through your father's lineage.
This is how your name will appear on your membership record. Please confirm this is correct before submitting.
